Blood. 2020;135(5):381\u2013386. doi:10.1182\/blood.2019002614.<\/i><\/a><\/p>\n<h3>R\u00e9sum\u00e9 de l\u2019article :<\/h3>\n<p>Au-del\u00e0 des risques de complications vasculaires, des donn\u00e9es r\u00e9centes mettent en lumi\u00e8re le risque pour les patients atteints de SMP de d\u00e9velopper un second cancer. Les auteurs pr\u00e9sentent ici les r\u00e9sultats d\u2019une \u00e9tude nich\u00e9e dans l\u2019\u00e9tude cas-t\u00e9moin internationale multicentrique MPN-K Study qui avaient inclus des patients atteints de SMP Ph- pr\u00e9sentant ou non un 2<sup>e<\/sup> cancer. Les \u00e9v\u00e9nements thrombotiques art\u00e9riels survenant apr\u00e8s le diagnostic de SMP (et avant le diagnostic de second cancer) \u00e9taient sup\u00e9rieurs dans le groupe SMP + second cancer par rapport aux t\u00e9moins : 6,2 % contre 3,7 % (p= 0,015). Apr\u00e8s ajustement, le risque de thrombose art\u00e9riel demeure associ\u00e9 de fa\u00e7on ind\u00e9pendante avec le risque de carcinome (RC 1,97 ; IC 95 % : 1,14-3,41).<\/p>\n<h3>Dans nos pratiques :<\/h3>\n<p>Des r\u00e9sultats qui poussent \u00e0 \u00eatre proactif en cas d\u2019\u00e9v\u00e9nement thrombotique art\u00e9riel chez un patient suivi pour un SMP. Les auteurs proposent de r\u00e9aliser un bilan similaire \u00e0 celui recommand\u00e9 par l\u2019ISTH en cas de thrombose veineuse non provoqu\u00e9e. Outre un examen clinique et un interrogatoire d\u00e9taill\u00e9, ce bilan peut comprendre une radiographie du thorax, un bilan biologique standard ainsi que des examens de d\u00e9pistage adapt\u00e9s \u00e0 l\u2019\u00e2ge et au sexe (mammographie, coloscopie, etc.).<\/p>\n<h3>Regard du statisticien :<\/h3>\n<p>Une \u00e9tude s\u00e9rieuse, bas\u00e9e sur des donn\u00e9es de qualit\u00e9 avec des effectifs notables. La m\u00e9thode d\u2019appariement, r\u00e9alis\u00e9e avec 3 t\u00e9moins pour un cas, assure une bonne prise en compte des facteurs de confusion potentiels. Les nombreux ajustements r\u00e9alis\u00e9s viennent encore limiter le risque de biais de confusion.
