Med. 384, 924\u2013935 (2021).<\/i><\/a><\/div>\n<div><\/div>\n<div>\n<h3><strong>R\u00e9sum\u00e9 :<\/strong><\/h3>\n<p>La recherche d\u2019anomalies g\u00e9nomiques constitue un \u00e9l\u00e9ment fondamental pour la stratification des LAM et des SMD. Les techniques de s\u00e9quen\u00e7age pourraient, dans le futur, concurrencer la cytog\u00e9n\u00e9tique classique. Les auteurs pr\u00e9sentent leur m\u00e9thode de s\u00e9quen\u00e7age complet du g\u00e9nome (WGS) appliqu\u00e9e \u00e0 265 patients au diagnostic. Parmi les 235 patients disposant d\u2019une analyse cytog\u00e9n\u00e9tique classique, l\u2019ensemble 40 translocations r\u00e9currentes ont pu \u00eatre correctement d\u00e9tect\u00e9es.<\/p>\n<h3><strong>Dans nos pratiques :<\/strong><\/h3>\n<p>Illustration de l\u2019utilit\u00e9 et de la fiabilit\u00e9 du WGS qui permet de concurrencer ici la cytog\u00e9n\u00e9tique standard en identifiant de nouvelles anomalies permettant parfois de modifier le classement pronostic. Un important travail d\u2019optimisation rendait possible l\u2019obtention des r\u00e9sultats avec un d\u00e9lai m\u00e9dian de 5 jours ! Gr\u00e2ce \u00e0 la baisse continue du co\u00fbt des techniques de s\u00e9quen\u00e7age, cette approche pourrait \u00e9galement rev\u00eatir un int\u00e9r\u00eat financier.<\/p>\n<h3><strong>Le regard du statisticien :<\/strong><\/h3>\n<p>En biostatistique, le remplissage des donn\u00e9es est un \u00e9l\u00e9ment-cl\u00e9 dans l\u2019obtention de r\u00e9sultats pr\u00e9cis et ici la m\u00e9thode WGS permet d\u2019apporter une classification plus nette des risques cytog\u00e9n\u00e9tiques. L\u2019exemple donn\u00e9 dans cette \u00e9tude (sur la survie des patients) permet de voir \u00e0 quel point la diff\u00e9rence entre les 2 m\u00e9thodes peuvent donner deux interpr\u00e9tations diff\u00e9rentes, la nouvelle \u00e9tant la plus juste.
