Med. 384, 1323\u20131334 (2021).<\/span><\/i><\/a><\/div>\n<div><\/div>\n<h3><strong>R\u00e9sum\u00e9 :<\/strong><\/h3>\n<div>\n<p>Les patients souffrant de la maladie des agglutinines froides (MAF) vont pr\u00e9senter une h\u00e9molyse caus\u00e9e par l\u2019activation de la voir classique du compl\u00e9ment. Le sutimlimab est un anticorps monoclonal qui cible la prot\u00e9ine C1s. Dans cet essai multicentrique, 24 patients atteints de MAF transfus\u00e9s ont \u00e9t\u00e9 inclus. Apr\u00e8s au moins une dose, 13 patients (54 %) pr\u00e9sentaient une r\u00e9ponse (h\u00e9moglobine &gt;12 g\/dL et\/ou +2 points).<\/p>\n<h3><strong>Dans nos pratiques :<\/strong><\/h3>\n<p>L\u2019inhibition s\u00e9lective de la fraction C1s du compl\u00e9ment semble prometteuse dans la MAF avec une diminution rapide de l\u2019h\u00e9molyse. Si le taux de r\u00e9ponse est similaire \u00e0 celui observ\u00e9 avec le rituximab en monoth\u00e9rapie, le sutimlimab pourrait constituer une nouvelle ligne chez les patients en \u00e9chec et\/ou ne pouvant b\u00e9n\u00e9ficier de fludarabine. La fonction immunitaire du compl\u00e9ment para\u00eet pr\u00e9serv\u00e9e.<\/p>\n<h3><strong>Le regard du statisticien :<\/strong><\/h3>\n<p>L\u2019\u00e9tude en question ressemble \u00e0 un essai de phase II, cela aurait \u00e9t\u00e9 un &#8216;plus&#8217; que ce soit inscrit dans le cadre d\u2019un v\u00e9ritable essai clinique.
