Allogeneic transplantation in advanced cutaneous T-cell lymphomas (CUTALLO): a propensity score matched controlled prospective study. <\/em><em>Lancet. 2023.<\/em><\/a><em><\/em><\/p>\n<p><strong>R\u00e9sum\u00e9 de l\u2019article<\/strong><\/p>\n<p>L\u2019\u00e9tude CUTALLO est un essai Fran\u00e7ais prospectif multicentrique randomisant l\u2019allogreffe de moelle osseuse (HSCT) contre traitements conventionnels dans les lymphomes T cutan\u00e9s (mycosis fungo\u00efde et syndrome de S\u00e9zary) de stade avanc\u00e9. Un total de 99 patients a \u00e9t\u00e9 inclus (55 et 44 dans les groupes HSCT et sans HSCT respectivement). En analyse appari\u00e9e par score de propension, l\u2019HSCT montrait un b\u00e9n\u00e9fice significatif sur la survie sans progression (m\u00e9dianes \u00e0 9,0 mois <sub>95%CI<\/sub>[6,6\u201330,5] contre 3,0 mois [2,0\u20136,3], p&lt;0,0001), qui \u00e9tait le crit\u00e8re de jugement principal. L\u2019HSCT \u00e9tait associ\u00e9e \u00e0 une am\u00e9lioration de la qualit\u00e9 de vie (mesur\u00e9e par EORTC, p=0,005), tandis que la mortalit\u00e9 li\u00e9e \u00e0 la greffe \u00e9tait de 19 % \u00e0 2 ans.<\/p>\n<p><strong>Dans nos pratiques<\/strong><\/p>\n<p>Le pronostic des lymphomes T cutan\u00e9s de stade avanc\u00e9 reste sombre. Les donn\u00e9es positives de cette premi\u00e8re \u00e9tude randomisant l\u2019HSCT dans contexte plaident fortement en faveur de son utilisation chez les patients pr\u00e9sentant un lymphome T cutan\u00e9e au stade avanc\u00e9 et en r\u00e9mission.<u><\/u><\/p>\n<p><u><br \/>\n<\/u><\/p>\n<p><u><\/u><strong>Le regard du biostatisticien<\/strong><\/p>\n<p><u>Attention<\/u> : il est question ici d\u2019un essai avec randomisation dite \u00ab g\u00e9n\u00e9tique \u00bb ou \u00ab mend\u00e9lienne \u00bb, c\u2019est-\u00e0-dire \u00ab qui suppose que la disponibilit\u00e9 d&#8217;un donneur agit comme une allocation al\u00e9atoire \u00e0 un groupe de traitement \u00bb.
