N Engl J Med 2023; 389:899-910. doi: 10.1056\/NEJMoa2300709.<\/i><\/a><\/p>\n<p><strong>R\u00e9sum\u00e9<\/strong><\/p>\n<p>Cette \u00e9tude rapporte l\u2019exp\u00e9rience de 3 patients porteurs de LAL-T multir\u00e9fractaires ayant re\u00e7u une infusion de BE-CAR7. Il s\u2019agit d\u2019un CAR T TCRa\/b-\/CD52-\/CD7- g\u00e9n\u00e9r\u00e9 par \u00e9dition de base utilisant la technologie CRISPR-Cas9 permettant notamment de limiter les recombinaisons chromosomiques en comparaison du CRISPR conventionnel. Ce traitement a conduit une r\u00e9mission chez les patientes #1 et #3 (13 ans et 15 ans) \u00e0 J28 permettant une allogreffe de moelle osseuse. Le patient #2 (13 ans) est d\u00e9c\u00e9d\u00e9 d\u2019une infection \u00e0 <em>Aspergillus niger<\/em>.<\/p>\n<p><strong>Dans nos pratiques<\/strong><\/p>\n<p>Il s\u2019agit d\u2019une \u00e9tude preuve de concept. Le BE-CAR7 permet notamment i\/ de produire des CAR-T disponibles rapidement g\u00e9n\u00e9r\u00e9s \u00e0 partir de donneurs universels ; et de pr\u00e9venir  \u00e0 la fois ii\/ le rejet des CAR T par une inactivation du CD52 (permettant en parall\u00e8le une lymphod\u00e9pl\u00e9tion T par alemtuzumab) ; iii\/ une fratricidie des CAR T par une inactivation du CD7 ; iv\/ et un effet GVH par une inactivation du TCR.<\/p>\n<p><strong>Le regard du biostatisticien<\/strong><\/p>\n<p>Dans ce type de publication, le regard ne se pose pas sur les tests statistiques (inexistants) mais particuli\u00e8rement sur les graphiques. Outre les repr\u00e9sentations classiques de cytom\u00e9trie en flux, on retrouve des histogrammes et des courbes d\u2019\u00e9volution en fonction du temps. Des d\u00e9tails ont \u00e9t\u00e9 apport\u00e9s sur les graphiques afin d\u2019apporter des indicateurs chronologiques suppl\u00e9mentaires par des frises (surfaces color\u00e9es), des ic\u00f4nes sp\u00e9cifiques (en plus de diff\u00e9rents codes couleurs ou style de traits), afin d\u2019apporter un niveau de lecture et informations suppl\u00e9mentaires aux graphiques.
